Job summary

Stellus Rx seeks a Population Health Pharmacist to join its Adhere Operations Team, helping to turn prescribed use into guaranteed health outcomes across populations.

You will work with Stellus Rx leaders to develop, implement, and oversee programs that improve medication therapies and adherence, leveraging data analytics and cross-disciplinary collaboration to benefit millions of Americans.

Qualifications

• PharmD from an accredited institution.
• Active pharmacist licensure in your home state and TX in good standing.
• Advanced training in population health management (preferred).
• Strong analytical and data interpretation skills.
• Experience in population health or managed care settings.

Responsibilities

• Develop, implement, and evaluate population health initiatives focused on medication management, adherence, and optimizing outcomes.
• Utilize health data analytics to identify trends and opportunities to improve medication-related outcomes for the target population.
• Collaborate with healthcare providers on medication selection, dosing, and monitoring per guidelines.
• Educate patients about medications to improve understanding and adherence and manage side effects.
• Coordinate care plans with physicians, nurses, social workers and other professionals.
• Participate in quality improvement projects to enhance outcomes and reduce adverse events.
• Stay updated with pharmaceutical regulations and ensure compliance with laws and guidelines.
• Provide leadership within the pharmacy team and mentor junior staff.
