Hawai‘i Pacific Health is seeking a Population Health Coordinator to support Hawai‘i Health Partners’ physician members with population health and quality metrics. You will analyze performance data to inform decisions and program implementation, and translate improvement gaps into actionable interventions.

You will collaborate with clinic operations, analytics, health IT, and senior leadership, while mentoring Population Health staff on complex assignments.

Competenze

• Bachelor’s degree in healthcare or related field OR equivalent combination of education, training, and experience.
• Current driver’s license and valid Hawaiʻi auto insurance.
• Two (2) years experience in health care, population health, OR equivalent experience in a healthcare setting.
• Coursework or certification in CPT and/or ICD-10 coding and/or related CPT/ICD-10 experience.
• Formal training or experience in performance improvement techniques (Lean, Six Sigma, 5S, Kaizen).
• One (1) year experience working with EPIC to optimize documentation for care delivery/quality.
• Two (2) years experience in a medical office.
• Two (2) years experience with Electronic Health Records (EHRs).
