Frontier Direct seeks an RN/LVN Nurse Coordinator for Population Health & Complex Care in Texas. You will drive population health outcomes, coordinate high-touch care, and prepare AI-assisted medical case review packets for Medical Director sign-off, partnering with Frontier clinic staff for patient care and outreach.

You will lead complex-case management, conduct rapid follow-ups after ER visits or hospitalizations, and ensure adherence to HIPAA and professional standards while supporting

Qualifikationen

• Active, unrestricted Texas LVN or RN license in good standing with the Texas Board of Nursing; 3+ years in case management, chronic disease or ambulatory care preferred.
• Proven track record in high-touch patient care and proactive engagement with complex patients.

Aufgaben

• Maintain protocolized registries for HTN, DM, asthma, CKD, and lipids.
• Proactively contact and schedule out-of-metric members for follow-up visits.
• Coordinate post-discharge follow-ups within 24–48 hours to reconcile care plans and prevent readmissions.
• Prepare templated pre-briefs and participate in weekly Complex Case Conferences with Medical Director.
• Track weekly action items to achieve ≥90% closure within 7 days.
