Job summary

Care-For-the-Homeless- is seeking a Health Educator to join the Population Health Management team in New York. You will develop and evaluate interventions to improve health outcomes and access to care, while coordinating educational events and community outreach to address population needs.

The role emphasizes developing programs, collaborating with CQI teams, and maintaining strong relationships with colleagues, clients, and providers. Bilingual English/Spanish is strongly preferred.

Qualifications

• Bachelor’s degree in Health Education and Health Promotion, Community Health, or Public Health required or strongly preferred.
• CHES certification preferred.
• Prior experience as Health Educator in a health centre setting.
• Experience developing, implementing, and evaluating health education programs.

Responsibilities

• Develop, implement, and evaluate targeted interventions to improve health outcomes and patient access to care, based on population data.
• Host targeted educational health events and conduct outreach.
• Create and implement health education activities and workshops for population needs.
• Use program planning and performance improvement models for development and evaluation.
• Participate in population health assessments and CQI activities.
• Increase awareness of health topics and available programs and services.
• Collaborate with community partners to implement programming.
• Coordinate community outreach to form partnerships and raise awareness.
• Engage in targeted outreach to close gaps in care using internal and managed care data.
