Job summary

The MVPP Clinical Manager at Boston Medical Center will lead the multidisciplinary care management program for high-utilization patients seen in the Emergency Department. You will supervise students, coordinate social services, and collaborate with ED, Population Health, Psychiatry, and other partners to improve outcomes and reduce unnecessary utilization.

You will engage community resources, practice harm-reduction approaches, and ensure thorough documentation in the EMR while guiding staff

Qualifications

Master’s in Social Work (MSW) required.
LICSW preferred or LCSW with relevant experience.
Minimum five years of experience in social services, healthcare, or public health.
At least one year of supervisory/management experience preferred.
Experience in emergency services and homelessness services preferred.

Responsibilities

Address unmet clinical and social needs of high-utilization ED patients.
Supervise and mentor students and ensure clinical supervision and training.
Build relationships with ED staff and health-system partners for care coordination.
Support care transitions, crisis planning, and documentation in EMR.
Engage with internal and external partners including city/state agencies and shelters.
