Job summary

Boston Medical Center is seeking a MVPP Clinical Manager to lead clinical services for a specialized care management program targeting super utilizers in the Emergency Department. You will collaborate with ED, Population Health, Psychiatry, and Grayken Center leadership to improve outcomes.

MSW with LICSW preferred; you will supervise BSW, MSW, and MPH students, coordinate care, and engage community partners while maintaining strict documentation and compliance standards.

Qualifications

• Master’s in Social Work (MSW) required; LICSW preferred.
• Active Massachusetts LICSW preferred; LCSW or MSW with licensure plans considered.
• Minimum five years in social services, healthcare, or public health.
• At least one year of supervisory/management experience preferred.
• Prior experience in emergency services and with high-risk populations preferred.
• Lived experience is highly valued.

Responsibilities

• Address unmet clinical and social needs of super utilizers through direct patient engagement, care coordination, crisis planning, and resource connection.
• Supervise, coach, and mentor students; provide clinical supervision and training.
• Build relationships with ED staff and other departments to meet patient needs.
• Support safety of high-risk patients via care planning, transitions of care, and case conferencing.
• Complete documentation in the EMR; ensure data integrity and MassHealth compliance.
• Engage internal/external partners and community agencies to support program goals.
• Assist leadership with program analysis and administrative tasks; contribute to strategy.
