Job summary

The Virginia Department of Health seeks a Population Health Community Coordinator for the Roanoke City/Alleghany Health District. This role focuses on developing and implementing environmental strategies to improve population health, representing the district at events, and coordinating health community activities with coalitions, schools, and partners.

The position supports education and engagement goals for prevention and wellness with youth, and may involve LGBTQ+ engagement, substance use

Qualifications

• Knowledge of Health Education & Promotion and population health data collection.
• Excellent oral and written communication to guide staff and stakeholders.
• Knowledge of procurement, budgeting, grant applications and reporting.
• Ability to build relationships with community partners and agencies.
• Ability to verify data, identify trends, and prepare reports to support planning.
• Proficiency with social media for health communication and standard office software.
• Ability to develop and manage project plans and timelines.

Responsibilities

• Serve as Healthy Communities Coordinator for the local health district’s environmental health strategies.
• Represent the health district at community events and coordinate health community activities.
• Support LGBTQ+ engagement, substance use prevention, and reproductive health initiatives.
• Provide community education and facilitate coalition meetings and events.
• Assist in conducting community health assessments and improvement planning.
• Promote collaboration and consistency across health programs.
• Develop partnerships to address health disparities and improve population health.
