Job summary

People Incorporated Mental Health Services in Saint Paul, MN seeks a Population Health Care Coordinator to advance integrated care across behavioral, physical, and mental health services in outpatient settings. You will coordinate care planning, engage clients, analyze population health data, and support teams with documentation in the EHR.

The role requires a master’s or equivalent in behavioral sciences, with substantial care coordination experience.

Qualifications

• Master’s degree in a behavioral science or related field.
• Bachelor’s degree in a behavioral science or related field with a practicum or internship that included direct work with children or adults.
• 30 college credits in behavioral sciences plus a practicum or internship with direct client interaction or 2,000 hours of relevant experience.
• High school diploma plus 4,000 hours of related service experience with mental illness/traumatic brain injury or developmental disabilities (with supervision until hours complete).

Responsibilities

• Integrated Client Care Coordination – coordinate care for clients with co-morbid conditions, ensuring continuity and monitoring across service domains.
• Coordinate and schedule Collaborative Care Planning meetings every 6 months or as needed.
• Assist clients with insurance renewal dates and support applying or reapplying for coverage.

Education

Master’s degree in a behavioral science or related field

Bachelor’s degree in a behavioral science or related field with practicum or internship

30 college credits in behavioral sciences + related experience or practicum

4,000 hours of experience with mental illness or related conditions (see options)
