Community Health Centers of America is seeking a Psychiatric Nurse Practitioner to provide in-person care at contracted skilled nursing facilities in the Burlingame and Concord areas. The role may include telehealth for other locations, with full-time or part-time options.

Candidates must hold a CA PMHNP license, a Master's degree, and appropriate certifications. You will diagnose and treat mental health conditions, prescribe medications, educate patients, and maintain accurate records while

Qualifications

• Must have CA PMHNP license and Master's degree.
• 3–5 years of related experience preferred.
• Current BLS/ACLS certification and DEA registration required.

Responsibilities

• Provide direct psychiatric medical services, including examination, diagnosis, treatment, referral, and follow-up.
• Prescribe medications and monitor treatment plans.
• Educate patients on diagnoses, treatment and prognosis.
• Maintain complete medical records per protocols.
• Participate in quality assurance and care coordination.
• Collaborate with interdisciplinary team and nursing staff to implement treatment/service plans.
