Job summary

Community Health Centers of America is seeking a Psychiatric Nurse Practitioner to provide in-person visits at Skilled Nursing Facilities in the Burlingame and Concord areas, with some telehealth options for other locations.

The role requires CA PMHNP licensing, a master’s degree, and 3-5 years of related experience preferred. You will diagnose and treat mental health conditions, write prescriptions, and educate patients and families while maintaining medical records.

Qualifications

• CA license as a PMHNP and NP.
• Master's degree in nursing or related field.
• 3–5 years of related experience preferred.

Responsibilities

• Provide psychiatric medical services including examination, diagnosis, treatment, and follow-up.
• Prescribe medications and educate patients on diagnosis and prognosis.
• Maintain complete medical records per protocols and HIPAA requirements.
• Coordinate care and participate in quality assurance activities.
• Educate patients and families about treatment plans and medications.
