Job summary

Telos Health Systems is seeking experienced PMHNPs to provide geriatric care in Skilled Nursing/Assisted Living settings. You will lead internal medicine services, collaborate with an interdisciplinary team, and oversee resident care across the facility.

The role supports a flexible 2+ days per week schedule and can be full-time if partnered with multiple nearby facilities. A hybrid care model combines in-person care with virtual charting and comprehensive benefits.

Qualifications

• PMHNP certification and active license with DEA required.
• Minimum one year of experience in a Skilled Nursing setting preferred.
• Ability to thrive in a fast-paced healthcare environment with strong communication.

Responsibilities

• Lead as a Skilled Nursing Facility provider of internal medicine services.
• Collaborate with interdisciplinary team and physicians.
• Oversee day-to-day care of residents with nursing staff and medical directors.
• Perform health assessments, differential diagnoses, and prescribe treatments.
• Prescribe medications and perform comprehensive physical examinations.
• Develop and implement patient management policies and procedures.
• Coordinate referrals to other healthcare professionals and facilities.
• Communicate with attending physicians and/or specialists about patient care.
• Maintain documentation and CPT coding proficiency.
• Conduct monthly stability visits for long-term residents.
• Evaluate changes in patient conditions and plan care accordingly.
• Support Advanced Care Planning.
