Job summary
CHE Behavioral Health Services seeks a licensed Mental Health Professional (PHYS) to provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities in California. Work will be part-time with flexible scheduling, allowing meaningful patient relationships while supported by an established multidisciplinary team.
Responsibilities include diagnostic assessments, treatment planning, and collaboration with facility staff.
Qualifications
- Active and unrestricted CA license as NP.
- Master's degree or doctoral degree in an applicable discipline.
- Ability to provide compassionate, patient-centered care.
- Strong communication and documentation skills.
- Comfortable providing services in nursing home and long-term care settings.
Responsibilities
- Provide in-person psychotherapy and behavioral health services within SNFs and Assisted Living communities.
- Conduct diagnostic assessments and develop treatment plans.
- Treat geriatric-related mental health concerns such as depression, anxiety, and trauma.
- Maintain timely and accurate clinical documentation.
- Collaborate with interdisciplinary care teams to improve patient outcomes.
- Build therapeutic relationships with residents and their families.
