Job summary

Geode Health is actively seeking Psychiatric Nurse Practitioners for both full-time and part-time roles. The ideal candidate is PMHNP certified, licensed to practice, and experienced with adult and/or child/adolescent populations.

Responsibilities include evaluations, follow-ups, urgent consultations, and collaboration with a care team to develop individualized treatment plans, while maintaining patient confidentiality and navigating EHR systems efficiently.

Qualifications

• Must hold current license/registration to practice.
• Excellent verbal and written communication skills.
• Must have ability to exercise sound judgment and discretion.
• Ability to maintain highly confidential information.

Responsibilities

• Provide psychiatric care, including evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs
• Document and navigate EHR independently
• Maintain strict client confidentiality
• Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information
• Evaluate patients for the need for a higher level of care, including hospitalization
• Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services
• Work closely with Supervising physician to ensure improved patient outcomes and high-quality care
• Diagnose and treat patients from different backgrounds/diverse backgrounds in a respectful manner
• Document and navigate EHR independently and in a timely manner.
• Work collaboratively with clinical leadership, billing, and compliance to ensure accurate and appropriate documentation.
