CASES in the Bronx, NY seeks a Psychiatric Mental Health Nurse Practitioner for a full-time, salaried role at 424 East 147th Street, Bronx, NY 10455. The FACT team provides integrated services for adults with serious mental illness and co-occurring needs.

You will conduct psychiatric evaluations, prescribe medications, coordinate care with nursing staff and partners, and complete required documentation. Salary ranges from $165,000 to $170,000 annually.

Qualifications

• Licensed as Psychiatric Mental Health Nurse Practitioner by NYS Education Department.
• Current board certification by AANP or ANCC.
• Extensive experience with adults with serious mental illness and co-occurring disorders.
• Excellent written and communication skills.

Responsabilités

• Conduct initial and monthly psychiatric evaluations in the community.
• Prescribe and educate about medications, monitor responses and side effects.
• Coordinate medication management with FACT nursing staff and labs as needed.
• Complete assessments, notes, incident reports and Medicaid billing docs.
• Coordinate care with families and community providers and collateral services.
• Participate in team and clinical meetings to discuss client needs.
• Apply motivational interviewing and trauma-informed approaches in services.
• Perform additional duties as required by ACT leadership.

Connaissances

Licensed PMHNP NYS

Board certification AANP/ANCC

Experience with adults with SMI

Excellent written & verbal skills

Bilingual Spanish
