Job summary

Aliya Healthcare is looking for an experienced Nursing Supervisor to lead nursing care on the assigned unit in our Illinois facility. You will ensure high-quality resident care, supervise staff, and participate in committees under the Director of Nursing.

The ideal candidate has 5–7 years in long-term care, an active Illinois RN license, and at least two years of supervisory experience. You will work with the interdisciplinary team to maintain regulatory compliance and deliver holistic care.

Qualifications

• Registered Nurse license in Illinois required.
• 5–7 years of long-term care nursing experience.
• Minimum of 2 years supervisory experience in acute, sub-acute or long-term care.
• Proficient in computer use and facility programs.

Responsibilities

• Assumes responsibility for resident care on the assigned unit.
• Report incidents promptly to Administrator and Director of Nursing.
• Participate in committee/programs as directed by DON.
• Oversee care to maximize residents' physical, mental and psychosocial functioning.
• Perform nursing assessments of residents' health/status.
• Communicate significant changes in condition to physicians and families.
• Make daily rounds to ensure care needs and environmental standards.
• Collaborate with interdisciplinary care team to provide holistic care.
• Review clinical records for completeness and accuracy.
• Monitor, track, evaluate and report infections.
• Serve as a resource for nursing staff.
• Review admissions with DON and Admissions Director.
• Coordinate orientation and continuing education for unit staff.
• Attend nursing supervisory meetings.

SUMMARY

Manages and assumes responsibility and accountability for resident care within the facility. Manages the unit in accordance with policy and procedure, State and Federal regulations to promote high quality care and service.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assumes responsibility and accountability for resident care on assigned unit.

• Ensures complete and prompt reporting of incidents with follow-up as necessary to Administrator and Director of Nursing.

• Actively participates in committee/programs as directed by Director of Nursing.

• Oversees resident care to promote the highest level of physical, mental and psychosocial functioning possible for assigned unit.

• Performs nursing assessments regarding the health/status of residents.

• Ensures that significant changes in resident condition are communicated to the physician, family or responsible party.

• Makes daily rounds on unit to ensure resident care needs and environmental standards are met, this includes monitoring of dining room during meal times.

• Works collaboratively with other members of the interdisciplinary care team to provide holistic care.

• Reviews clinical records for completeness and accuracy as necessary.

• Monitors, tracks, evaluates and reports infections.

• Acts as a resource for nursing staff.

• Reviews applications for admission to the unit with Director of Nursing and Admissions Director.

• Works collaboratively with Director of Nursing to identify and provide orientation and continuing education for unit staff members.

• Attends and actively participates in nursing supervisory meetings.

• Other duties as assigned.

QUALIFICATIONS

Graduation from an accredited School of Nursing plus 5-7 years of experience as a long-term care nurse; of which, a minimum of two years supervisory experience in acute, sub-acute or long-term care. Must be licensed as a Registered Nurse in the State of Illinois. Must be proficient in the use of computer and programs utilized by the facility.
