Job summary

Aliya of Palatine is seeking a dedicated Licensed Practical Nurse (LPN) / Registered Nurse (RN) to join our skilled nursing facility care team for the PM shift. You will provide total nursing care to residents and supervise team members, ensuring compliance with facility standards.

You will participate in developing individualized care plans with the interdisciplinary team, administer medications, and monitor resident conditions.

Qualifications

• Graduate of an accredited nursing education program.
• Active Illinois LPN or RN license in good standing.
• No prior abuse convictions.
• Previous skilled nursing, rehabilitation, or long-term care experience preferred.
• Strong communication, organizational, and teamwork skills.
• Ability to follow facility policies, maintain resident safety, and deliver compassionate, resident-centered care.
• Ability to work effectively with diverse resident populations, including elderly and cognitively impaired individuals.
• Ability to perform the essential functions of the position with or without reasonable accommodation.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN/RN.
• Participate in developing and implementing individualized care plans with the interdisciplinary healthcare team.
• Administer medications and treatments as prescribed and evaluate their effectiveness.
• Maintain an environment that promotes residents' physical, social, emotional, and psychological well-being.
• Educate residents and families regarding care plans, medications, treatments, and procedures.
• Monitor nursing team member performance and assist with coaching, development, evaluation and corrective action when appropriate.
• Delegate nursing tasks appropriately and ensure assigned duties are completed accurately and timely.
• Remain current on facility policies, infection prevention practices, and continuing nursing education.
• Recognize, assess, and promptly respond to significant changes in resident condition.
• Accurately document resident care, treatments, observations, and responses in accordance with facility policies and regulatory requirements.
• Serve as Charge Nurse by assisting with clinical decision-making during the shift.
• Follow all facility safety policies, infection control standards, and regulatory requirements.
• Collaborate with physicians, therapists, and other team members to provide exceptional resident care.
