Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team in Illinois. The role focuses on providing total nursing care, supervising team members, and ensuring compliance with facility standards.

The LPN will participate in care planning, administer medications, and educate residents and families, while remaining committed to high‑quality resident care and professional development.

Qualifications

• Graduation from an accredited nursing education program.
• Active Licensed Practical Nurse license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills are essential.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Develop and implement individualized care plans alongside the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents’ physical, social, and psychological well‑being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures.
