Job summary

St. Rose Hospital in Hayward, CA is seeking a Registered Nurse for the Skilled-Subacute unit on the PM shift.

This full-time role provides direct patient care and requires accurate communication with physicians, the Nurse Director, and coworkers about changes in a patient’s condition and diagnostic results. The RN will perform medication administration with two patient identifiers, document per policy, participate in medication reconciliation, and ensure crash cart readiness.

Qualifications

• Current California RN license required.
• BCLS upon hire and maintained.
• Minimum six months acute care hospital or long-term care experience.

Responsibilities

• Use two patient identifiers when drawing labs, medications, and procedures.
• Do not use abbreviations not approved; read back telephone orders.
• Follow medication reconciliation policy and document medications accurately.
• Ensure crash cart readiness and replace items after codes with signature.
• Comply with Infection Control policies and PPE usage.
• Provide thorough shift handoffs and patient education on discharge as needed.
