Job summary

St. Rose Hospital in Hayward, CA is seeking a full-time Registered Nurse for the Skilled-Subacute unit.

The nurse provides direct patient care, communicates changes in condition with physicians and the care team, and ensures timely documentation and standards compliance. Qualifications

• Current and valid RN license issued by the State of California.
• BCLS on hire and maintain current.
• Six months experience in Acute Care hospital or Long Term Care.

Responsibilities

• Provide direct care in the Skilled-Subacute unit.
• Communicate changes in patient condition with physicians, Nurse Director and co-workers.
• Ensure accurate documentation, including MAR and narcotic sheets.
• Adhere to infection control policies and PPE usage.
• Assist with procedures and tests, and use time-outs for bedside procedures.
• Prepare discharge planning and patient/family education.
