Comprehensive Rehab Consultants seeks an NP/PA/Physician Assistant to deliver PM&R services at designated SNFs in Portsmouth, NH and Bedford, NH. This W-2, 32-hour position emphasizes autonomy, collaboration with therapy teams, and improving patient independence in post-acute care settings.

The ideal clinician will have NH licensure, U.S. work eligibility, and post-acute experience, with generous salary and bonuses. Training, education opportunities, and benefits are included.

Qualifications

• Experience in post-acute space preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with leadership and collaboration with care team.
• Collaborate with PCP teams for management of SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
