Job summary

Comprehensive Rehab Consultants seeks a PM&R Clinician (NP/PA) to deliver physiatry services in designated SNFs in Portsmouth or Bedford, NH. The role emphasizes autonomy, collaboration with rehab teams, and a strong commitment to geriatric post-acute care.

Qualifications

• NP/APRN/PA license required/active in NH.
• New Hampshire medical license required for practice in NH.
• Ability to work in the U.S. (visa status)
• Post-acute care/post-acute space experience preferred.
• Desire to deliver high-quality patient care with empathy and professionalism.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF or post-acute centers.
• Represent CRC with clinical leadership in your facility.
• Collaborate with PCP teams on SNF conditions (CVA, neurodegenerative, postoperative ortho).
• Work with PT/OT/SLP teams to improve function and quality of life.
• Follow patients to track progress and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Participate in multidisciplinary meetings and bridge therapy with internal medicine.
