Job summary

Hickory Grove Post Acute in Gallatin, Tennessee is seeking a licensed practical nurse (LPN) to join our evening/afternoon shift team on a full-time basis. The LPN will provide high-quality patient care to residents, administer medications, monitor vital signs, and collaborate with RNs and other healthcare professionals.

The PM shift runs from 7 PM to 7 AM with alternating rotations. Compensation is $36-$39 per hour, reflecting a commitment to staffing and quality care.

Responsibilities

• Provide direct patient care: assist with ADLs, administer medications, and record vital signs.
• Monitor patient conditions and report changes to the RN or physician and assist with care plans as needed.
• Collaborate with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate records of patient information, medications, and vital signs in accordance with facility policies.
• Participate in quality improvement initiatives to improve patient outcomes and facility processes.
