Hickory Grove Post Acute in Gallatin, Tennessee is seeking a licensed practical nurse (LPN) to join our evening/afternoon shift team on a full-time basis. The LPN will provide high-quality patient care to residents, administer medications, monitor vital signs, and collaborate with RNs and other healthcare professionals.

The PM shift runs from 7 PM to 7 AM with alternating rotations. Compensation is $36-$39 per hour, reflecting a commitment to staffing and quality care.

Hickory Grove Post Acute is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening/afternoon shift team at our Gallatin, Tennessee facility. Hickory Grove is a 124-bed facility that has recently undergone some remodeling. We are committed to staffing our building well so that our healthcare workers can deliver the highest quality of care.

Pay: $36-$39 per hour.

Shift: PM (7PM to 7AM) Alternating Rotations

Responsibilities

• Provide direct patient care: Assist with activities of daily living (ADLs), administer medications, and record vital signs to ensure the health and well-being of our residents.
• Monitor patient conditions: Report any changes in patient status to the Registered Nurse (RN) or physician and assist with implementing care plans as needed.
• Collaborate with healthcare team: Work closely with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate records: Accurately document patient information, medications, and vital signs in accordance with facility policies and procedures.
• Participate in quality improvement initiatives: Contribute to improving patient outcomes and facility processes through active participation in quality improvement activities.

Requirements

Working Relationship

• The LPN will report directly to the Registered Nurse (RN) or Director of Nursing.
• Collaborate with other healthcare professionals, including physicians, physical therapists, occupational therapists, and speech therapists.
