Responsibilities

• Provide nursing care to patients in a post-acute setting, including administering medications and wound care
• Assist with activities of daily living (ADLs)

• Monitor vital signs and report changes
• Maintain accurate patient care records
• Collaborate with healthcare professionals to ensure seamless care transitions
