Job summary

Oakwood Gardens Care Center in California is seeking a Licensed Vocational Nurse (LVN) for the Full-Time PM shift. The role provides direct nursing care, supervises CNAs, administers medications, and updates resident records.

Ideal candidates are graduates of an approved LVN/LPN program with an active California LVN license and CPR certification. Long-term care experience is preferred; PCC knowledge is a plus and strong communication and teamwork are essential.

Qualifications

Graduate of an approved LVN/LPN program.
Long-term care or skilled nursing experience preferred.
Current California LVN license and CPR certification.
Knowledge of PCC preferred.
Strong communication and leadership abilities.

Responsibilities

Provide direct nursing care in accordance with resident assessments and care plans.
Supervise and support CNAs to ensure quality care delivery.
Administer medications and treatments as ordered by physicians.
Accurately document nursing care, medication administration, and resident status.
Monitor residents for changes in condition and report findings promptly.
Participate in admissions, transfers, and discharges as assigned.
Ensure compliance with infection control, safety, and sanitation standards.
Communicate effectively with residents, families, physicians, and interdisciplinary team members.
Assist with restorative and rehabilitative nursing programs.
Respond appropriately to emergencies and resident needs.
Maintain resident confidentiality and comply with HIPAA regulations.
