Job summary

Oakwood Gardens Care Center is seeking a Licensed Vocational Nurse (LVN) for the AM shift on a full-time basis. You will provide direct nursing care to residents, supervise CNAs, and ensure care plans are followed in accordance with regulations.

The role requires an LVN license in California and CPR certification; additional PCC knowledge is a plus. You will document care, administer medications, monitor residents, and participate in admissions and discharges while maintaining HIPAA compliance.

Qualifications

• Graduate of an approved LVN/LPN program.
• Long-term care or skilled nursing experience preferred.
• Current California LVN license and CPR certification.

Responsibilities

• Provide direct nursing care per assessments and care plans.
• Supervise CNAs to ensure quality care.
• Administer medications and treatments as ordered.
• Document nursing care, medication administration, and resident status.
• Monitor residents for changes and report findings promptly.
• Participate in admissions, transfers, and discharges as assigned.
• Ensure infection control, safety, and HIPAA compliance.
• Communicate with residents, families, physicians, and team members.
• Assist with restorative and rehabilitative nursing programs.
• Respond to emergencies and resident needs.
