Job summary

Signature Healthcare at Home in Washington is seeking a Home Health Physical Therapist to support patients in the Pierce County/Puyallup area. This PRN/Part Time role follows a day shift schedule, 8:00 AM to 5:00 PM, with an hourly rate of $50-$60 depending on experience. Mileage is paid between patients.

You will provide rehabilitative PT in patients' homes, develop and update the plan of care with patients and caregivers, and educate families. Travel mileage is reimbursed as needed.

Qualifications

• Licensed to practice as a physical therapist in WA.
• Degree from a CAPTE-approved PT program (baccalaureate or master).
• Two years of PT experience; home health preferred.
• Excellent verbal/written communication and organization.
• CPR certification current; valid driver’s license and reliable vehicle.

Responsibilities

• Provide rehabilitative PT in patients' homes per physician plan of care.
• Develop and update the care plan with patient and caregivers.
• Counsel patients and families; educate on therapies and devices.
• Prepare notes and progress summaries; coordinate with physicians.
• Supervise PT assistants per policy; participate in discharge planning.
• Offer in-service education and collaborate with care teams.
