Signature Healthcare at Home in Pierce County offers PRN/Part Time Home Health Physical Therapist position. The role covers patients in their homes across the Puyallup area, with a day schedule and mileage reimbursement.

You will provide skilled PT services, develop care plans, and educate patients and families while coordinating with the healthcare team. The position requires WA licensure, CAPTE-accredited degree, CPR, and driving access.

Formación

• Licensed to practice as a physical therapist in WA State.
• Two years of PT experience; home health preferred.
• Current CPR certification.

Responsabilidades

• Provide rehabilitative and skilled physical therapy services in patients' homes.
• Develop and adjust individualized plans of care with patients and caregivers.
• Educate patients, caregivers, and families on treatment and home exercises.
• Assess functional status, gait, strength, and ROM; document progress.
• Communicate with physicians and health-team members about care plans.
• Supervise aides as appropriate and participate in discharge planning.
