Summary : Holy Cross Hospital Jordan Valley West (HCW) is looking for additional OB / GYN's to fill their call coverage needs. The call coverage is for concurrent coverage at HCW and HCS, OBGYN will cover the call coverage in the OBGYN group.
Rate Caps for Submission :
Malpractice - Must be included in rates.
Orientation -
Holiday - Must be 1.5x the hourly rate

Assignment : 7 shifts per month. The call includes 2 sites.

Required Procedures / Job Duties : General OB / GYN procedures & job duties. Candidates will need to see patients in clinic, provide deliveries and procedures in the hospital, and group call coverage.

Reason for Coverage : Departure of existing physician and another physician ramping down in preparation for retirement.

EMR Required Training :
Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training.
EMR Virtual training - Only able to attend if Modules are completed before training. Provider is required to schedule and complete their virtual EMR training prior to their first day, to avoid missing their first shift.

Travel Preferences :
Preferred Lodging : N / A- use standard GSA rate
Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information :
Timeframe : 60-120 days
Requirements : UT license must be in place to release preapplication. UT license, CSR, and UT DEA must be in place to release full application.

Census / patient population : Adult, Adolescent, Geriatric
What is the anticipated Patient Volume per shift? Call volumes - 2-3 patient calls per month for every 7 days of call.
What does call look like if covering call at 2 locations? Is there a backup physician that will take call if the primary physician is onsite at the other location? GYN emergencies only at SL, very few. 1 every couple of months. Family med takes most things there. SL does not have a month / baby unit.
SUBMISSION REQUIREMENTS
Board Certification OB / GYN - Required or Truly BE (0-5 years of residency completion)
Active UT License - Required or IMLC (Must have LOQ at time of submission)
Active BLS or PALS, & Controlled substance (CSR) - Required for credentialing
Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
Candidate Must be available for a minimum of 7 shifts per month - Required (Must note on submission)
Candidate must be available for either of the June stints in job description, to be considered for ongoing dates - Required
Candidate must acknowledge they understand that call coverage is at 2 sites (concurrent coverage) - Required
Candidate Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
Clean Malpractice / background - Highly Preferred, Candidates who present with a self inquiry NPDB report will be placed with high priority.
