If you're tired of restrictive office settings, enjoy the flexibility of community-based work at Skilled Nursing and Assisted Living Facilities. Lead a team of Advanced Practice Providers and collaborate with an interdisciplinary team including Psychiatry, Psychotherapy, Podiatry, and Wound Care where available.

Responsibilities:

• Provide primary care services following site-specific protocols and standards for family medicine, geriatric medicine, and/or internal medicine in long-term care facilities.
• Review and sign MARs, sign all orders, review labs within 24 hours, and manage scheduled medication prescriptions per policy.
• Allocate adequate time for paperwork and administrative duties during visits.
• Participate in care plan meetings and QA/PI meetings as requested.
• Be available between 8AM and 5PM on scheduled days via text or phone; triage policies will be updated as needed.
• Submit documentation and billing records according to schedules.
• Maintain open communication with facility staff and adhere to standards, policies, and legal regulations, including HIPAA and CPT guidelines.

Qualifications:

• Licensed MD or DO
• Preferably 3 years in Long Term Care/Geriatric Care
• AMDA Certification is a plus
• Strong leadership and organizational skills
• Excellent communication skills
