Work Environment and Responsibilities:

Enjoy the flexibility of community-based work at Skilled Nursing and Assisted Living Facilities, leading a team of Advanced Practice Providers and collaborating with interdisciplinary teams including Psychiatry, Psychotherapy, Podiatry, and Wound Care.

Detailed Duties and Responsibilities:

• Provide primary care in accordance with site-specific protocols and standards of family medicine, geriatric medicine, or internal medicine related to long-term care facilities.
• Review and sign MARs, sign all orders, and manage labs and medication prescriptions as per policies.
• Allocate adequate time for paperwork and administrative duties during visits.
• Participate in care plan meetings and QA/PI meetings as requested.
• Be available for on-call duties between 8AM and 5PM, with support from our ECare team; schedule and policies may evolve.
• Submit documentation and billing records timely.
• Maintain open communication with facility staff and administration, providing instruction and feedback as needed.
• Adhere to company standards, policies, legal, and regulatory guidelines, including HIPAA and CPT coding.

Qualifications and Education:

• Licensed MD or DO
• Preferably 3 years in Long Term Care or Geriatric Care
• AMDA Certification is a plus but not mandatory
• Strong leadership and organizational skills
• Excellent communication skills
