Are you tired of the typical restrictive office-based setting? Enjoy the flexibility of community-based work in Skilled Nursing and Assisted Living Facilities. Lead a team of Advanced Practice Providers and collaborate with an interdisciplinary team, including Psychiatry, Psychotherapy, Podiatry, and Wound Care (where available).

Responsibilities include:

• Providing primary care in accordance with site-specific protocols and standards for family medicine, geriatric medicine, and internal medicine in long-term care settings.
• Reviewing and signing MARs, signing orders for supervising physicians, managing labs and medication prescriptions as per policies, and addressing lab results within 24 hours.
• Allocating sufficient time for paperwork and administrative duties during visits.
• Participating in care plan meetings and QA/PI meetings as requested.
• Being available for on-call duties between 8AM and 5PM, with schedules supported by the ECare team, adhering to triage policies.
• Submitting documentation and billing records according to schedule.
• Maintaining open communication with facility staff, providing instructions, and meeting with administrators daily.
• Adhering to standards, policies, legal, and regulatory guidelines, including HIPAA and CPT coding.

Qualifications/Education:

• Licensed MD or DO
• Preferably 3 years in Long Term Care/Geriatric Care
• AMDA Certification is a plus but not mandatory
• Strong leadership and organizational skills
• Excellent communication skills
