Twin Cities Physicians is a provider-led medical group delivering on-site primary care and geriatric services across senior living communities throughout Minnesota. Our model focuses on preventative care, chronic disease management, continuity of care, and reducing avoidable hospitalizations. By bringing care directly to assisted living, memory care, and long-term care communities, we help patients receive the medical attention they need in the place they call home.
Many Physician Assistants entered healthcare because they wanted to spend time with patients, think critically, and make meaningful clinical decisions.
Over time, healthcare can become increasingly focused on schedules, volume, and administrative demands.
This opportunity offers something different.
As a Physician Assistant with Twin Cities Physicians, you will provide on-site care throughout senior living communities while managing chronic conditions, coordinating care, and helping patients navigate complex healthcare needs. You'll have the opportunity to build continuity with your patient population while practicing in a setting where your clinical judgment truly matters.
This role is ideal for providers seeking a rewarding practice environment that combines geriatric medicine, preventative care, chronic disease management, and community-based healthcare.
