This role is pivotal in integrating specialized geriatric care across the hospitals most complex clinical pathways, ensuring that our older adult population receives age-sensitive, evidence-based interventions.
The successful candidate will act as a key collaborator with diverse multidisciplinary teams, bridging the gap between acute specialized care and the unique physiological and psychosocial needs of older adults.
