Responsibilities

Seeking a full-time physician trained in Geriatrics, Internal Medicine, or Family Medicine to serve as the primary care provider for medically complex seniors in the PACE model. This is a collaborative, patient-centered position where you'll work closely with a multidisciplinary team to coordinate and deliver comprehensive care.

• Setting: Outpatient, nursing home, and home care coordination
• Schedule: Full-time; no call or weekends required
• Team Collaboration: Daily IDT meetings, integrated care planning

Responsibilities Include:

• Primary care visits and chronic condition management
• Acute illness evaluations
• Nursing home rounding and care coordination
• Integration of specialty care and hospital oversight
• Quality improvement through data review and clinical collaboration

Qualifications
