We are hiring a full-time physician to perform primary care rounding in two skilled nursing facilities (SNF) and one assisted living facility (ALF) in the Wilmington, Delaware area. Job Duties

• Perform primary care rounding in SNF and ALF settings
• Manage chronic diseases, diagnose illnesses, and implement care plans
• Collaborate with clinicians, facility leaders, and staff regarding patient care and services
• Build relationships with patients, their families, and facility staff to facilitate effective treatment
• Oversee, guide, and mentor nurse practitioners
• Conduct clinical case reviews and share relevant tools, information, and guidelines
• Represent TeamHealth in external community engagements within post-acute care
• Work with leadership on care coordination initiatives and network management
