Job summary

Trumen Physicians + Associates is seeking a Post-Acute Care Physician to join a physician-led organization on a flexible, independent contractor basis in Beaumont, TX. You will care for a panel of patients in skilled nursing and rehabilitation facilities, focusing on reducing readmissions and guiding transitional care with continuity.

Enjoy autonomy in clinical decision-making, a full patient panel through established facility relationships, and tax flexibility as a 1099 contractor.

Qualifications

• MD or DO with a current, unrestricted Texas medical license.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• 1+ year of post-acute, geriatric, or SNF care experience preferred.
• Comfortable practicing independently in skilled nursing facilities.
• Strong clinical judgment and a patient-centered mindset.
• Reliable transportation to assigned facilities.

Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities.
• Perform assessments, manage treatment plans, and document in the EMR.
• Collaborate with facility teams to support quality outcomes and transitions of care.
• Collaborate in Trumen's value-based care and transitional care programs.
• Develop strong working relationships with facility and clinical staff.
• Serve as a liaison between Trumen and facility teams.
• Maintain an on-site presence consistent with your agreed schedule.

About the Role:

As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities. In this role, you’ll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.
