Job summary

Trumen Physicians + Associates is seeking a Post-Acute Care Physician to manage a panel of patients in skilled nursing and rehabilitation facilities in Beaumont, TX. You will collaborate with facility staff and Trumen clinicians to reduce readmissions and guide patients through transitional care.

As an independent contractor, you will have autonomy in clinical decision-making and access to coordination resources, with a flexible schedule tailored to your life and patient panel.

Qualifications

• MD or DO with Texas medical license required.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• 1+ year of post-acute, geriatric, or SNF care preferred.

Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities.
• Perform assessments, manage treatment plans, and document in EMR.
• Collaborate with facility teams to support quality outcomes and transitions of care.
• Coordinate within Trumen's value-based and transitional care programs.
• Maintain on-site presence per scheduled facility needs.

About the Role

As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities. In this role, you’ll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.

Core Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities
• Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR
• Collaborate with facility teams to support quality outcomes and smooth transitions of care
• Collaborate as applicable in Trumen's value-based care and transitional care programs
• Develop strong working relationships with facility and clinical staffServe as a liaison between Trumen and facility teams
• Maintain an on-site presence consistent with your agreed schedule and facility needs

Qualifications

• MD or DO with a current, unrestricted Texas medical license
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
• 1+ year of experience in post-acute, geriatric, or SNF care preferred
• Comfortable practicing independently in skilled nursing facilities
• Strong clinical judgment, communication skills, and a patient-centered mindset
• Reliable transportation to assigned facilities
