Job summary

Trumen Physicians + Associates invites a Post-Acute Care Physician to join a physician-led team in Lufkin, TX. You will manage a panel in skilled nursing and rehabilitation facilities, focusing on reducing readmissions and guiding transitional care with continuity.

The role offers autonomy, alignment with your schedule, and robust support from a provider-centric organization. Texas license and board eligibility in IM/FM/Geriatrics are required, with prior SNF experience preferred.

Qualifications

• MD or DO with an unrestricted Texas medical license.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• 1+ year of post-acute, geriatric, or SNF care experience preferred.
• Ability to practice independently in skilled nursing facilities.

Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities.
• Perform assessments, manage treatment plans, and maintain EMR documentation.
• Collaborate with facility teams to support quality outcomes and transitions of care.
• Engage in Trumen’s value-based and transitional care programs as applicable.
• Develop strong working relationships with facility and clinical staff.
• Serve as a liaison between Trumen and facility teams.
• Maintain an on-site presence per the agreed schedule and facility needs.

About the Role

As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities. In this role, you’ll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.

Core Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities
• Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR
• Collaborate with facility teams to support quality outcomes and smooth transitions of care
• Collaborate as applicable in Trumen’s value-based care and transitional care programs

• Maintain an on-site presence consistent with your agreed schedule and facility needs

Qualifications

• 1+ year of experience in post-acute, geriatric, or SNF care preferred
• Comfortable practicing independently in skilled nursing facilities

• Reliable transportation to assigned facilities
