Job summary

Trumen Physicians + Associates in Texas is seeking a Post-Acute Care Physician to provide patient care in skilled nursing and rehabilitative facilities with a flexible schedule.

You will work as an independent contractor (1099), partnering with facility staff and Trumen clinicians to reduce hospital readmissions, manage chronic and acute conditions, and guide patients through transitional care with continuity and compassion, all while maintaining autonomy and tax flexibility.

Qualifications

• MD or DO with a Texas license (unrestricted).
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• At least 1 year of post-acute, geriatric, or SNF care experience.
• Ability to practice independently in skilled nursing facilities.
• Strong clinical judgment and patient-centered approach.
• Reliable transportation within assigned facilities.

Responsibilities

• Provide in-person medical care to patients in post-acute facilities.
• Assess, manage treatment plans, and document in EMR.
• Collaborate with facility teams to improve outcomes and transitions.
• Participate in value-based care and transitional care programs.
• Develop relationships with facility and clinical staff.
• Serve as liaison between Trumen and facility teams.
• Maintain on-site presence per agreed schedule.

About the Role:

As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities. In this role, you’ll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.

Core Responsibilities:

• Provide in-person medical care to patients in designated post-acute facilities
• Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR
• Collaborate with facility teams to support quality outcomes and smooth transitions of care
• Collaborate as applicable in Trumen’s value-based care and transitional care programs
• Develop strong working relationships with facility and clinical staff
• Serve as a liaison between Trumen and facility teams
• Maintain an on-site presence consistent with your agreed schedule and facility needs

Qualifications:
