Job summary

Southeast Primary Care Partners seeks a Physician Assistant or Nurse Practitioner to join a collaborative clinic team. The APP will provide both chronic and acute care within a primary care setting, supporting physicians, RN Case Managers, and staff.

Qualifications

• 2+ years of primary care experience.
• Current licensure through the Georgia Board.
• Current prescriptive authority from the Georgia Board.
• BLS certification from the American Heart Association.

Responsibilities

• Provide care consistent with licensure and certification.
• Diagnose and treat acute and chronic conditions.
• Order and interpret diagnostic tests (labs, x-ray, EKG).
• Prescribe medications and other treatments.
• Counsel patients on disease management and outcomes.
• Collaborate with a multidisciplinary team to deliver care.
