Job summary

Health Solutions in Southern Colorado is seeking a Physician Assistant to join our Family Medicine team at the Perry Hall location. This full-time role provides the full scope of primary care for adults and children, including preventive care, diagnoses, and care coordination.

You will work under the Medical Services leadership, perform exams and tests, order imaging and labs, prescribe medications, and collaborate with physicians, nurses, and behavioral health colleagues to deliver integrated,

Signing bonus available!What You’ll Be DoingThe Physician Assistant is a provider of the full scope of primary care services which fall under primary care of adult and pediatric populations, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance for this population. In addition, the Primary Care Physician Assistant is a member of the Health Solutions Medical Services Team. In that capacity, they are expected to be familiar with the philosophy, goals, and objectives of the Team and work collaboratively to achieve them. They must be cognizant of, and comply with, all Center policies and procedures, as well as pertinent state and federal regulations.RESPONSIBILITIES: As a Physician Assistant, you will play a central role in delivering integrated, patient-centered care and APM performance metrics. Under the supervision of the Manager and Senior Vice President of Medical Services, the essential functions and other duties include:Clinical CareProvide comprehensive primary care across the lifespan, including health assessments, physical exams, diagnostic evaluations, and management of acute and chronic conditions using evidence-based guidelines.Order, perform, and interpret diagnostic tests (e.g., blood work, imaging, EKGs, urinalysis) to support accurate diagnoses.Develop and adjust individualized treatment plans, prescribing medications, therapies, and preventive interventions as appropriate.Deliver preventive services such as wellness visits, vaccinations, screenings, and routine check-ups.Engage patients in shared decision-making and self-management support; educate on disease prevention, healthy habits, and self-care strategies.Prepare and administer prescribed medications, injections, and vaccinations.Evaluate behavioral health assessments and refer to behavioral health professionals as needed.Perform office-based procedures within scope of practice and participate in acute care situations (e.g., urgent triage).Care Integration & CollaborationParticipate in daily huddles, case conferences, and care planning meetings.Coordinate referrals and care transitions across multiple providers and healthcare settings.Quality & ComplianceMaintain accurate, timely electronic health records and documentation supporting medical necessity, quality metrics, and compliant billing; collaborate with billing and quality teams as needed.Deliver whole-person orientation, coordinated care, enhanced access, and continuous quality improvement.Participate in quality initiatives and use data to drive performance on APM measures (e.g., patient engagement, preventive screenings, chronic disease outcomes).Meet productivity, patient access, and panel management benchmarks aligned with organizational and payer requirements.Adhere to confidentiality standards, medical ethics, and Colorado statutes related to collaborative practice and prescriptive authority.Professional DevelopmentStay current with medical research and guidelines through continuing education and integration of evidence-based practices.Advance the education of healthcare team members to promote high-quality care.Physical Requirements:Must be able to remain on feet for prolonged periods while evaluating and treating patients.Must be able to remain on feet for prolonged periods while evaluating and treating patients.Must be able to perform physical exams, clinical procedures, and operate medical instruments with precision.Must have adequate vision (with or without correction) to read medical records, observe patient conditions, and perform diagnostic procedures.Sufficient hearing (with or without accommodation) to effectively communicate with patients and staff and assess health sounds (e.g., heart, lung).Must be able to work in environments with potential exposure to communicable diseases, bloodborne pathogens, and other hazardous substances, while adhering to appropriate safety protocols and use of personal protective equipment (PPE).Must be able to move rapidly and react appropriately in urgent or high-stress clinical scenarios.What You’ll Like About UsCompetitive pay of $125000.00 to 137,000.00 annually (Dependent on experience, certification and shift). Signing bonus of $5,000 after successful completion of orientation, $2,500 after successful completion of 1st year of service, and $2,500 after successful completion of 2nd year of service.Generous benefits package.
