Job summary

Johns Hopkins Community Physicians in Baltimore is seeking a Nurse Practitioner or Physician Assistant to join a Lifestyle Medicine clinic. You will address root causes of chronic disease through the six pillars of lifestyle medicine and work with a multidisciplinary team to support patients in weight management and chronic disease care.

Qualifications

• CRNP or Physician Assistant license required
• CPR certification required
• Certification in Lifestyle Medicine and/or Obesity Management required
• 3 years’ experience in primary care, chronic disease management, or weight management preferred

Responsibilities

• Join Johns Hopkins Lifestyle Medicine clinic as Nurse Practitioner or Physician Assistant.
• Provide care addressing root causes of chronic disease through nutrition, activity, sleep, stress management, and other pillars.
• Prescribe weight-management medications when appropriate and coordinate with dietitians and care team.

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner or Physician Assistant to join our Lifestyle Medicine clinic. Johns Hopkins Community Physicians serves Maryland, Virginia and Washington DC with over 40 locations.

Put your passion to care towards our role as a Hopkins Lifestyle Medicine Provider where you'll help patients address the root causes of chronic disease through the six pillars of lifestyle medicine: nutrition, physical activity, stress management, restorative sleep, connectedness, and avoidance of risky substances. You'll use motivational interviewing and SMART goal setting to support lasting behavior change, and prescribe medication for weight management, including GLP-1s, when appropriate. You'll work closely with registered dietitians, medical assistants, and our primary care and specialty colleagues, who will rely on you to support their patients with chronic disease and weight management needs.

Requirements

• CRNP or Physician Assistant License in the State of Maryland, Virginia, and/or Washington DC required

• Certification in Lifestyle Medicine and/or Obesity Management required
• 3 years’ experience in primary care, chronic disease management, or weight management preferred
