Job summary

Cape Cod Hospital seeks an Advanced Practice Provider to lead postoperative care in the Cardiac Surgery program, from pre-operative readiness through ICU management and discharge planning. The role emphasizes hemodynamics, chest tubes, IABP knowledge, and collaboration with the surgical team.

This leadership position requires a Master's degree, NCCPA certification, DEA registrations, and BLS/ACLS, with 3-5 years' experience preferred.

Qualifications

• Graduate of a graduate-level PA program approved by the AMA.
• Active NCCPA certification.
• US DEA registrations required for prescribing practitioners.
• 3–5 years’ experience preferred.

Responsibilities

• Make daily rounds post cardiac surgery and report findings to the attending surgeon.
• Manage and implement care plans as directed, in conjunction with the attending surgeon, and autonomously.
• Direct and oversee other members of the surgical care team to support post- and pre-operative cardiac care.
• Manage hemodynamics and vasoactive medications.
• Handle postoperative drain management, chest tubes, and IABP knowledge.
• Perform wound care and invasive procedures for critically ill cardiac surgery patients (central lines, thoracentesis).
• Maintain patient care with ongoing reassessments, write orders, interpret tests and imaging, and obtain consultations as needed.
• Provide continuity of care from pre-operative to discharge planning.
• Daily progress notes, history and physicals, consult notes, and discharge summaries.
• Educate and assist other team members in patient progression and care planning.
• Demonstrate leadership by overseeing a patient’s care plan and directing team members as directed by the attending surgeon.
• Attend surgical staff meetings, grand rounds, and morbidity/mortality conferences.
• Maintain and enhance clinical knowledge by attending meetings and reviewing literature.

Job description

Purpose of Position: The position requires care throughout the Cardiac Surgery patient’s journey from initial consult to pre-operative readiness, to a heavy emphasis on post-operative ICU care with management of hemodynamics including swan catheters, central lines and chest tubes. Additionally, the position is responsible for care in the cardiac surgery step-down unit as the patients transition toward discharge. Our program is at the forefront of minimally invasive valve programs. This will have the advanced practice provider working dynamically in our cardiac surgery team with a great scope of practice while providing leadership.

Description

• Makes daily rounds independently or with surgical team on patients post cardiac surgery and reports findings to the attending surgeon.
• Manages and implements care plans as directed by the attending surgeon in conjunction with the attending surgeon and autonomously
• Direct and oversee other members of the surgical care team so as to facilitate post and pre-operative cardiac surgical care.
• Responsible for managing hemodynamics along with vasoactive medications.
• Responsible for post-operative drain management and removal, along with chest tube insertion, intra-aortic balloon pump knowledge and management.
• There is emphasis on post-operative wound management as well as procedures needed for the care of the critically ill cardiac surgery patient including insertion of central lines and thoracentesis.
• Maintains patient care with ongoing re-assessments for evaluate changes in patient status, writes orders, interprets tests and imaging, and obtains consultations when needed.
• Provides a continuum of care from pre-operative care through critical care to discharge planning.
• Responsible for daily progress notes, history and physical consult notes and discharge summaries.
• Educate and assist other members of the surgical team in the care and progress of the patient’s pre and post-operative course.
• The advance practice provider is a leadership position and must demonstrate the ability to oversee a patient’s care plan and work autonomously and direct other members of the care team as directed by the attending surgeon
• Must attend surgical staff meeting, grand rounds, morbidity and mortality conference.
• Maintain and enhance skill and clinical knowledge with clinical competence providers are expected to attend internal and external educational meetings and keep current with appropriate professional journals/literature.
• Ability to work rotating days, nights and weekends
• Perform other work-related duties as requested/directed

Schedule Details: 72PP (36 hrs. per week), Rot. Days, 7p-7a or 7a-7p, weekend and holidays coverage required in rotation. (Cardiac Surgery OR Endoscopic skills preferred.)
