Job summary

Duly Health and Care Elmhurst is seeking an Advanced Practice Provider (APP) to deliver high-quality care with autonomy and collaboration across a robust network. Enjoy a strong internal referral network, access to resources, and a supportive team spanning Internal Medicine and more than 1,200 providers.

The compensation package includes a base salary range of 100k–140k plus potential sign-on bonuses, comprehensive benefits, CME funds, 401K match, and generous PTO.

Qualifications

• Graduation from an approved school for Physician Assistant or Nurse Practitioner with a Master's degree.
• Licensed or ability to obtain licensure in the state of Illinois.
• Internal Medicine or similar setting experience preferred (1+ year).

Responsibilities

• Practice at the top of license, forming trusted relationships with patients and providers.
• Collaborate with physicians and care teams to coordinate high-quality patient care.
• Contribute to a supportive, efficient care environment within a large provider network.
