Job summary

Erie Family Health Centers is seeking an Advanced Practice Provider to deliver comprehensive primary care at Erie Westside Health Center in Chicago, IL. You will perform exams, diagnose, treat, and refer to specialists as needed, while maintaining productivity targets.

The position requires a Master’s degree in a related field, active IL PA/APRN license, board certification, DEA license, and BLS. Experience in an FQHC setting and Spanish language skills are preferred.

Job Title: Advanced Practice Provider

Location: Erie Westside Health Center at Ward - 646 N. Lawndale - Chicago, IL 60624

Salary Range: $110,000.00 - $125,000.00

Job Overview

The Advanced Practice Provider will provide comprehensive primary health care service to patients in the prevention, diagnosis, and treatment of disease. Acts as a primary health care provider with the scope of primary care specialty, performs physical examinations on new and returned clients, refers patients to specialists as necessary, documents patient care activities, and maintains a productivity level necessary to reach mandated annual level of encounters.

Main Duties & Responsibilities
  • Performs history and physical on new/return patients throughout the lifespan.
  • Orders appropriate diagnostic/screening tests and reviews results to establish diagnosis; initiates or continues treatment based on findings.
  • Manages acute and chronic illness according to established protocols and/or in collaboration with physicians.
  • Provides follow-up health care referrals or community agencies as necessary.
  • Maintains a productivity level necessary to reach mandated annual client encounters.
Qualifications & Education
  • Master of Medical Science or M.S. in Physician Assistant Studies or an MSN from an accredited program is required; DNP from an accredited program is preferred.
  • Active PA or APRN license in IL or eligible for IL licensure.
  • Board certification.
  • Active DEA and Controlled Substance Licenses or eligible for DEA and Controlled Substance Licenses.
  • Active CPR (BLS) certification.
Experience
  • Minimum of two (2) years in primary care practice; or one (1) year of practice as an FQHC primary care provider; or completion of an APP primary care Fellowship program post‑graduation.
  • Experience serving in an FQHC, Community Health Center, or working with traditionally underserved patient populations is highly preferred.
  • Ability to utilize interpretation services for patient encounters is required. Fluency in Spanish or a language in addition to English is strongly preferred.

  • Commitment to increasing number of APRNs working in underserved communities and providing high‑quality compassionate patient care.
  • Comfort with providing clear and constructive feedback.
  • Energetic, deadline‑driven, self‑starter.
  • Excellent organizational and project management skills.
  • Working knowledge of PC using Microsoft products such as Word and PowerPoint in a Windows operating environment.
  • Ability to develop and maintain excellent working relationships with preceptors, partners, clinical staff, managers, and directors.
  • Interest in and comfort with working with diverse patient population.Knowledge of or interest in learning medical Spanish.
Compensation

Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
