Endeavor Health in Elmhurst IL seeks an Advanced Practice Provider (APN/PA) to join the Internal Medicine Department at Elmhurst Health Center. The APP will see new and established patients, orders tests, interprets results, and guides treatment under a collaborating physician.

Full-time, with flexible hours and on-call rotation. The role requires IL APN/PA licensure, DEA and IL Controlled Substance license, BLS/CPR certification, and experience in outpatient settings.

Qualifications

• APN/PA licensure in IL required; DEA and IL Controlled Substance License required.
• BLS/CPR certification required.
• Outpatient or community health setting experience preferred.

Responsibilities

• Provide care to patients under supervision of collaborating physicians.
• Perform history and physical examinations; order imaging and labs; interpret results.
• Educate patients on treatment plans and post-operative instructions.
• Maintain records and progress notes; follow patients through care in clinic and hospital.
• Collaborate with health care team to achieve optimal outcomes and safety.
