Job summary

Chestnut Health in Illinois seeks a dedicated Physician Assistant to join our Physiatry/PM&R team. You will collaborate with therapists, nurses, and physicians to develop comprehensive rehabilitation plans, perform histories and exams, and monitor progress to optimize functional outcomes.

New graduates are encouraged to apply. Active PA license in Illinois preferred; DEA license and CS restrictions required on start date.

Qualifications

  • Eligible to work in the United States.
  • Active Physician Assistant license in Illinois preferred.
  • Current Controlled Substance License preferred (required upon start date).
  • Current DEA License preferred (required upon start date).
  • NCCPA Certificate preferred.
  • Interest in Physiatry/PM&R preferred.
  • Orthopedic Experience preferred.
  • Physiatry Experience preferred.
  • Skilled Nursing Experience preferred.

Responsibilities

  • Collaborate with therapists, nurses, and other healthcare professionals to develop rehabilitation plans.
  • Conduct patient assessments, histories, exams to determine rehab goals.
  • Monitor progress and adjust treatment plans to optimize outcomes.
  • Participate in multidisciplinary meetings and education activities.
  • Document evaluations, plans, progress notes, and discharge summaries in EMR.
  • Educate patients and families about rehabilitation and post-discharge care.

Essential Duties & Responsibilities:
  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
Qualifications:
