Qualifications

Active license to practice in the state of employed
Current Controlled Substance License required on start date
Current DEA License required on start date
NCCPA Certification
Interest in Physiatry/PM&R
Orthopedic experience a plus
Physiatry experience a plus
SNF experience a plus

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
Provide education and counseling to patients and families regarding rehabilitation process, outcomes, and post-discharge care.
