Job summary

Chestnut Health in Rockwood, TN seeks a Physician Assistant to join its Physiatry team. You will help develop rehabilitation plans with therapists and nurses, perform patient assessments, and monitor progress to improve independence and function.

Qualifications

• Actively licensed to practice in the state of employed
• Current Controlled Substance License (required on start date)
• Current DEA License (required on start date)
• NCCPA Certification
• Desire to work in Physiatry/PM&R
• Orthopedic experience is a plus
• Physiatry experience is a plus
• Skilled Nursing experience is a plus

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR.
• Provide education and counseling to patients and families regarding rehabilitation and post-discharge care.

• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
